Dominion House Case Manager

Responsibilities Development of individual service plans for the 12 adults diagnosed with mental illness who live in a stabilization group home in Schenectady, NY. Providing supportive, goal‑oriented counseling with a focus on decision‑making, problem‑solving skills, and crisis prevention and intervention. Teaching daily living skills, including medication supervision and medication management, and facilitating socialization and recreational activities. Maintaining a healthy, clean environment to support client independence, including cleaning and general upkeep. Coordinating regular service planning meetings with clients and primary counselors. Assisting clients in applying for benefit entitlements, connecting them to funding sources such as DSS and SSA, and advocating on their behalf with other agencies. Completing financial paperwork, entering pertinent information on case‑management notes, and communicating regularly with Finance to request client checks and reconcile accounts. Requirements Associate’s degree in human services (BA preferred). At least 1 year of case‑management experience. Excellent communication and organizational skills. Ability to multi‑task and follow through. Valid and insurable driver’s license. Benefits 40 hours per week. $23.77 per hour; $49,441.60 annual equivalent.
