Intake Social Worker - Hybrid

Benefits Medical, Dental, and Vision Insurance PTO - Paid time off 401k retirement plan Job Summary The Intake Social Worker’s role is to effectively engage, screen, assess, refer, provide brief interventions, and coordinate appropriate out-of-home placements for young people aged 0–23 who contact or are referred to Tabor Children’s Services. The Intake worker will need to be skilled in assessing youth’s needs, including risk assessments, and have the ability to create an action plan to mitigate any identified risks. The Intake Social worker will participate in regular supervision and will be required to present and discuss cases with the resource department. The Intake Social Worker works as part of a multidisciplinary team, including but not limited to, independent living staff, resource parents, recruiters, and resource parent support staff. The Intake Social Worker will need to build a positive rapport with Tabor Children Services’ resource parents as well as their peers in the field. The Intake Social Worker will coordinate placement referrals, intakes, and discharges during normal business hours. This position will complete reports on intakes and discharges and work closely with all team members to ensure intake and discharge timelines are met and the PA DHS 3700 regulations are always followed. Additionally, this position will assist staff by aiding in documentation and making contact with youth and resource parents. Non-traditional hours may occur due to the need for evening and weekend training for foster parents, recruitment events, on call, etc. Works a Hybrid schedule: 3 days in office and 2 days WFM (schedule can change based on departmental needs). Hours are from 9 am – 5pm Monday through Friday but may also vary according to departmental needs. Salary: Starting at $22.60 / hour Responsibilities Conducts field calls from DHS CRU and other outside counties/parties from 9:00 a.m. – 5:00 p.m. and coordination of placements for Tabor Children’s Services Philadelphia and surrounding counties Foster Care and Supervised Independent Living programs. Reviews all reports and recommendations issued by licensing and recruitment staff. Creates on call calendars, participates in on call on a rotational basis, and coordinates with on-call staff on a daily basis. Submits daily placement availability to DHS. Verifies and updates all client information and enters it into the electronic client case file. Tracks intake and discharges for all Tabor Children’s Services, Philadelphia and surrounding counties Foster Care and Supervised Independent Living programs. Obtains relative referral documentation and confirmation of placements for case records. Alerts staff to upcoming intakes and discharges for all programs. Assists in coordinating discharges as needed. Processes case updates and change requests accurately and timely at the direction of the program supervisors. Assists in coordinating travel arrangements for intakes, including coordinating meet and greets with resource parents. Coordinates and attends arranged intakes and ensure that all staff, clients, and resource parents are aware of the intake process. Attends case support tasks, which include (but are not limited to): filing, inputting information into relevant computer programs, case noting, completing phone calls to resource parents and intake departments, completing follow-up calls periodically, and conducting exit surveys with youth and resource parents. Manages referrals related to clients at risk of 30 or 90-day removal notices and determine appropriate intervention strategies and document them as implemented; ensures inappropriate referrals are channeled correctly and documented. Maintains current documentation and records of intake services, as well as statistical data and reports required by the program and supervisor. Works with the supervisor to schedule and prioritize work duties. Attends training as required by the supervisor. Other duties as assigned. Qualifications Bachelor’s degree. Degree in human services preferred. Two (2) years’ experience working in a Social Services environment. Strong written and verbal communication skills Skills with Microsoft Office applications, including Word, Excel, and PowerPoint Knowledge of PA Code 55 3680s, 3700s, and the Philadelphia DHS Foster Care Scopes of Service Knowledge and skills in cultural diversity Ability to achieve desired performance outcomes Ability to assess and solve problems Capable of working responsibly with highly confidential information Ability to multi-task and demonstrate good organizational skills Ability to work independently as well as part of a team Strong oral and written communication skills, and customer service skills Valid driver’s license and the ability to use your own vehicle. Clean driving history record. Must be free from any communicable or infectious diseases. Provide a valid PA child abuse clearance, FBI check, and criminal background check.
