Full-Time Trauma-Informed Community Case Manager

The Salvation Army in Omaha, NE is seeking a full-time case management professional to support community stabilization through in-home and mobile services. You will perform need-based assessments, develop and monitor service plans, and coordinate with families and supports while following trauma-informed practices. Daily duties include conducting assessments, creating ISP plans, ensuring regular contact, monitoring medications, and assisting with community resources to promote independence and #J-18808-Ljbffr KrocQuincy
