Position Summary

The MDS Nurse (RN) is responsible for coordinating the Resident Assessment Instrument (RAI) process, completing accurate and timely Minimum Data Set (MDS) assessments, and collaborating with the interdisciplinary team to develop comprehensive, individualized care plans. This position plays a critical role in supporting regulatory compliance, quality outcomes, reimbursement accuracy, and person-centered care while ensuring residents receive services that reflect their clinical needs.

Key Responsibilities

• Coordinate and complete accurate, timely MDS assessments in accordance with CMS, Medicare, Medicaid, and state regulations.
• Collaborate with the interdisciplinary team to develop, implement, and update individualized resident care plans.
• Review clinical documentation to ensure assessment accuracy, regulatory compliance, and appropriate reimbursement.
• Monitor MDS schedules and ensure all assessments are completed within required timeframes.
• Analyze resident conditions and documentation to accurately capture clinical complexity and functional status.
• Participate in Medicare meetings, care conferences, quality assurance activities, and interdisciplinary team meetings.
• Educate nursing staff on documentation requirements and best practices that support accurate resident assessments.
• Assist with survey preparation and maintain compliance with CMS, state, and facility standards.
• Identify opportunities to improve documentation, quality measures, and reimbursement outcomes.
• Foster a collaborative, resident-centered approach to assessment, care planning, and clinical excellence.

Requirements

Required Qualifications

• Current Registered Nurse (RN) license in the State of Kentucky.
• Current CPR certification (or ability to obtain within required timeframe).
• Minimum of two (2) years of nursing experience in skilled nursing, long-term care, rehabilitation, or a related healthcare setting.
• Working knowledge of the MDS/RAI process, care planning, and Medicare/Medicaid regulations.
• Strong understanding of clinical documentation and reimbursement principles.
• Excellent communication, collaboration, and organizational skills.
• Ability to manage multiple priorities while meeting regulatory deadlines.
• Strong attention to detail and commitment to documentation accuracy.
• Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
• Ability to meet the physical demands of the position, including lifting up to 50 pounds, prolonged standing and walking, bending, and extended periods of computer work and documentation.

Preferred Experience

• Previous experience as an MDS Coordinator or MDS Nurse in a skilled nursing or post-acute care setting.
• RAC-CT certification or willingness to obtain.
• Experience with PDPM, Medicare reimbursement, Quality Measures, Five-Star Quality Rating System, and regulatory compliance.
• Experience with electronic medical records (EMR) and MDS software.
• Knowledge of Quality Assurance and Performance Improvement (QAPI) processes.
• Strong analytical, problem-solving, and critical thinking skills.
