Advanced Practice Provider

Department: Diabetes & Cardiometabolic Health Reports to: Medical Director, Cardiometabolic Health Reporting to the Medical Director, the Advanced Practice Provider (APP) will serve as an essential clinician within our Diabetes & Cardiometabolic Health Center of Excellence – an innovative, interdisciplinary care model that unites endocrinology, cardiology, nephrology, and ancillary services to deliver integrated, wrap-around care for patients with complex cardio-kidney-metabolic disease. The APP provides longitudinal, evidence-based management of diabetes, obesity, hypertension, dyslipidemia, cardiovascular disease, chronic kidney disease, and related metabolic disorders, partnering closely with physicians, registered dietitians, certified diabetes care and education specialists, pharmacists, care managers, behavioral health providers, and primary care teams to improve outcomes, slow disease progression, and deliver coordinated, patient-centered care aligned with Premium Health’s mission. Responsibilities Conduct comprehensive new patient evaluations, consultations, follow-up visits, and chronic disease management across Endocrinology, Cardiology, and Nephrology. Develop, implement, and adjust individualized treatment plans using evidence-based clinical guidelines and validated risk calculators (e.g., PREVENT) to guide risk stratification and therapy intensification. Order, interpret, and trend laboratory studies, diagnostic testing, imaging, and specialty evaluations. Initiate, titrate, and monitor medications including insulin, GLP-1 receptor agonists/dual incretin therapies, SGLT2 inhibitors, antihypertensives, lipid-lowering therapies, and guideline-directed medical therapy (GDMT) for heart failure and chronic kidney disease. Interpret continuous glucose monitoring (CGM) reports, ambulatory blood pressure monitor (ABPM) reports, and other remote patient monitoring data to optimize treatment plans and close care gaps Identify patients at elevated cardiovascular and renal risk and implement preventive strategies to reduce disease progression and complications. Provide patient education regarding diabetes self-management, nutrition, exercise, weight management, medication adherence, blood pressure control, cardiovascular risk reduction, and chronic kidney disease management. Coordinate care with Primary Care, Endocrinology, Cardiology, Nephrology, Registered Dietitians, Certified Diabetes Care and Education Specialists (CDCES), Behavioral Health, Pharmacy, Care Management, and external specialists to ensure seamless transitions of care. Monitor and improve clinical quality metrics including A1c, blood pressure, LDL cholesterol, BMI, eGFR, urine albumin/creatinine ratio, medication adherence, and other organizational quality measures. Support value-based care initiatives, HEDIS, UDS, chronic disease registries, and population health outreach efforts. Respond to patient messages, refill requests, urgent clinical concerns, and provider escalations in a timely manner. Maintain timely, accurate documentation in the electronic medical record. Participate in departmental care team huddles, case conferences, quality improvement initiatives, protocol development, and clinical workflow optimization. Provide cross-site coverage across Premium Health locations as operational needs require. Adhere to the highest standards of clinical care and comply with all regulatory, accreditation, and organizational policies. Qualifications Current unrestricted New York State Nurse Practitioner or Physician Assistant license. Current DEA registration and BLS certification. 2-3 years of clinical experience in Endocrinology, Cardiology, Nephrology, Internal Medicine, Family Medicine, Obesity Medicine, or related chronic disease management strongly preferred. Experience working in an FQHC, ambulatory care, or value-based care environment preferred. Strong knowledge of diabetes, obesity, cardiovascular disease, hypertension, chronic kidney disease, and evidence-based cardiometabolic therapies. Excellent interpersonal, written, and verbal communication skills. Ability to exercise sound clinical judgment and maintain highly confidential information. Strong organizational, documentation, and time-management skills. Ability to collaborate effectively within a multidisciplinary team.
