Licensed Practical Nurse

Licensed Practical Nurse (LPN) Currently looking for EXPERIENCED Licensed Practical Nurse(s) (LPN) to fill multiple openings with a home care agency located in Raritan, NJ. Interested candidates should have a minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility. Responsibilities of the Licensed Practical Nurse (LPN) * Provide thoughtful care to client & patients based on assessment and personalized Plan of Care * Monitor and track change in the client/patient’s condition and needs, keep the Director of Nursing and any other appropriate personnel informed * Proactively update Director of Nursing on health changes and concerns * Pay rate: $24- $35hr., depending on assignment Requirements of the Licensed Practical Nurse (LPN) * Current unrestricted LPN license in New Jersey * Pediatric experience preferred * Current BLS certification * Negative PPD or chest x-ray * Minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility * Licensed driver with valid automobile insurance * Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. * Varied assistance is required, depending on the mobility of the client. * Must be able to lift up to 50 pounds *We are an Equal Opportunity Employer and do not discriminate against applicants due to race, ethnicity, gender, veteran status, or on the basis of disability or any other federal, state or local protected class.
