- In-Home Assessments : Conduct thorough evaluations of patients' physical, cognitive, and emotional abilities in their home environment, tailoring therapy plans to each individual's needs.
- Therapeutic Program Development : Design and implement therapy strategies to enhance mobility, fine motor skills, self-care abilities, and daily living functions.
- Education & Training : Educate patients and their caregivers on exercises, adaptive tools, and techniques that support rehabilitation and promote greater independence at home.
- Team Collaboration : Partner with physicians, nurses, and other healthcare professionals to ensure integrated and comprehensive care for each patient.
- Accurate Documentation : Keep detailed records of patient progress, treatment sessions, and outcomes to ensure compliance with home health care guidelines.
