Primary Care Physician

Job Overview Conviva Primary Care, a subsidiary of Humana Inc., is looking for a Primary Care Physician at our clinic in Deerfield Beach, Florida. The role involves hands‑on patient care, addressing social determinants of health, and focuses on senior‑focused primary care. Responsibilities Report to the Associate Medical Director. Work onsite and in‑person at the South Daytona clinic. Provide outpatient medicine, continuity of care, health maintenance, and disease prevention. Maintain medical histories and records. Refer patients to specialists. Influence clinic department strategy. Make clinical decisions on moderately complex to complex issues with latitude in best approaches. Participate in Humana/Senior Bridge’s tuberculosis screening program (TB screening required). Schedule: Monday‑Friday, 8:00 am–5:00 pm, see 12‑15 patients per day with 45‑minute appointments; 100 % of time clinically focused. Required Qualifications Doctor of Medicine or Osteopathic Medicine. Board certified or board eligible in Geriatric Medicine, Family Medicine, or Internal Medicine. Valid, unrestricted license in the state of desired employment (or willingness to obtain). Current, unrestricted DEA license. Preferred Qualifications Minimum two years of experience. One year experience managing Medicare Advantage panel. Knowledge of Medicare guidelines and coverage. Knowledge of HEDIS quality indicators. Fluency in Spanish and English. Compensation & Benefits Base salary: $255,000 – $298,500 per year (full time, 40 hours per week). Eligible for a bonus incentive plan based on company/individual performance.
