Community Health Advocate & Care Navigator

Humana seeks a Community Health Worker to serve as a liaison between health and social services and the community from downtown Atlanta to Decatur, GA. You will report to the Chief Medical Officer for the Atlanta Market and perform home visits, conduct needs assessments, and coordinate access to healthcare and social supports.
