Registered Nurse MDS Coordinator

We are seeking a highly skilled and compassionate MDS Coordinator to join our team at The Avenue at Westshore, a premier new skilled nursing facility in Holland, Michigan. As a key member of our team, you will play a vital role in shaping the future of care in our luxury-style environment.

About the Role:

As a Registered Nurse MDS Coordinator, you will be responsible for:

Coordination and Management : Oversee the completion of Minimum Data Set (MDS) assessments, ensuring accurate and timely documentation in accordance with regulatory requirements.
Clinical Expertise : Utilize your nursing expertise to develop and implement individualized care plans that meet the unique needs of our residents.
Communication and Collaboration : Work closely with interdisciplinary teams, physicians, and other healthcare professionals to ensure seamless care delivery and optimal resident outcomes.
Leadership and Guidance : Mentor and train staff members on MDS assessment and care planning best practices.
Quality Assurance : Participate in quality improvement initiatives to ensure compliance with regulatory standards and optimization of care processes.

Requirements:

License and Certification : Active RN license in the state of Michigan with current certification in nursing or related field.
