Grafton - Crit Care: IU 2nd Floor ICU

• Unit Information:


o Beds: 17


• Census? Average patients/day?


o Minimum Years of Experience required? 3 years' ICU experience


o Will you accept a first-time traveler? Yes


• Patient Types/Common Diagnoses:


o General: sepsis, ETOH, IVDA, OD, SI, delirium, GI bleeds, strokes w/ TPA, ARF, COVID, DKA


o Surgical: general, open abdominal, ortho (total joints), ENT, OBGYN


o Cardiac/Vascular: fempops, CABG, EVAR, valves, post cath (radial and femoral approach), AAA, amputations


o No complex neuro


• Patient Ratios:


o 1:2, based on acuity


• Required License, National Certification, Certs:


o BLS, ACLS; NIHSS preferred


• Skills required:


o Vent/trach management, invasive line management (art lines, CVP, Swanz), drains, chest tubes, foley management, post-surgical recovery, post-cath experience, CVVH, CRRT


o Preferred: cardiovascular and CV surgery experience, IABPs, Impellas


• Support within the Department:


o CNA - 2 during day, 1 at night


o Receptionist/HUC 0700-1930


o Charge nurse with occasional patient assignment


o PM ICU Supervisor Monday - Friday


o Phlebotomy for peripheral IV draws


o Educator


o RT 24/7


o Hospitalist 24/7


o Intensivist 24/7


o ICU specific pharmacist for days


• Technology/Equipment:


o EMR: EPIC - preferred


o IV Pump: Alaris


o Med dispensing: Pyxis


o Monitors:


o Unit specific equipment: NxStage CRRT


• Floating: If yes, where?


o Could float within scope of practice and below, including to intermediate units (1:3), MSU, ED Boarding (1:4), Observation units (1:4)


o Travelers first to float


• Orientation (facility and unit) :


o 1 Day of orientation:


• 0800 meet with educator (badge access, CBLs, login access, etc.)


• Meet and Greet with Nurse Manager


• Remainder of the shift with a preceptor


• Scheduling:


o Weekend rotation: every other


o On call? No


o Holiday Expectations: 1 per contract


o Approval of RTO: to go through manager


o Shift times: 0700-1930, 1900-0730


o Schedule cycle: made in 4-week increments

o More than 2 sick calls will prompt discussion with traveler and evaluation of contract and possible contract termination
