We are seeking a compassionate and skilled LPN (Licensed Practical Nurse) to join our team at Vantage at New Bedford, a leading provider of long-term care and skilled nursing services. About the Role:
As an LPN at Vantage, you will be responsible for:
Providing direct patient care to our residents, including administering medications, wound care, and treatments
Monitoring and reporting changes in residents' conditions to nursing staff and physicians
Collaborating with the healthcare team to develop and implement individualized care plans
Maintaining accurate and detailed records of residents' care and progress
Participating in the implementation of individualized care plans and providing support to residents and their families
