Title: RN – Home Health / Nurse Assessor

Location: New York, NY 10005

Pay: $2,339/week ($680 taxable + $1,659 tax-free stipend)

Schedule: Day Shift – Hybrid (2 days remote + 3 days in-field visits)

Contract: 5 years

Required Qualifications:

• Current New York State RN license
• Minimum 2 years of home-based nursing experience within the last 10 years
• Recent experience conducting comprehensive patient assessments
• Experience may include CHHA, LHCSA, MLTC, OASIS/UAS-type assessments, homebound patient care, hospice, assisted living, or home/community-based care
• Strong communication, organizational, documentation, and clinical assessment skills
• Ability to work independently in a fast-paced, deadline-oriented environment
• Comfortable using technology and electronic documentation systems

Key Responsibilities:

• Conduct initial in-home assessments of patients, including health status, care needs, strengths, and preferences
• Review medical records and referral documentation
• Enter and transmit assessment information electronically
• Identify and document concerns, conflicting information, and care gaps
• Develop and support individualized long-term care service plans
• Educate consumers regarding healthcare options, support services, vendors, and waiver programs
• Coordinate with interdisciplinary teams to improve care delivery and continuity
• Document case notes and collect required quality-review data
• Identify opportunities for health promotion and illness prevention
• Maintain knowledge of community resources, payer requirements, and available services
