Act as liaison, advocate, and coordinator for client involvement in self‑advocacy, transportation services, integrated community activities, recreation, and learning independent living skills when appropriate for the evening shift. Develop relationships with each client and their conservators to help organize and navigate visitation and passes for client leaves within the protocol. Foster communication for a smooth transition and teamwork between shifts. Oversee the client’s medical, medication, and health issues as needed. Provide and ensure communication with “all who need to know,” such as the Program Director, Nurse, staff, and families, informing of any pertinent issues.
