You’ll collaborate with physicians, families, and the interdisciplinary care team to deliver high-quality, individualized therapy services.### **Key Responsibilities*** Perform **patient evaluations** and develop individualized treatment plans* Provide therapy for **speech, language, cognition, and dysphagia*** Monitor resident progress and adjust treatment plans as needed* Maintain accurate and timely **clinical documentation*** Communicate effectively with residents, families, physicians, and care team members* Participate in **care planning and discharge planning*** Contribute to a positive rehab team culture and outcomes-focused care### **Qualifications*** Degree in **Speech-Language Pathology*** Active, unencumbered **SLP license in South Carolina (required)*** **Advanced degree preferred*** Experience in a **long-term care or rehab setting preferred*** Familiarity with **PointClickCare (PCC) and Casamba preferred*** Strong clinical, communication, and organizational skills* Commitment to resident-centered care and professional growth### ### ### **Why Join Patewood Post Acute?**At Patewood, you’re not just filling a role—you’re joining a team that invests in your success and offers real opportunities to grow into leadership.
