Mental Health Case Manager

Volunteers of America Delaware Valley

Volunteers of America Delaware Valley seeks a compassionate Mental Health Case Manager to support individuals with mental health needs and justice-involved backgrounds. You will conduct assessments, develop person-centered service plans, and coordinate access to housing and healthcare within a collaborative treatment team. In this role, you’ll monitor progress, adjust care plans, and provide group sessions and community outreach while ensuring compliance with funder guidelines and program #J-18808-Ljbffr
