Physical Therapist - Los Molinos, CA

Make a meaningful impact in the lives of patients by providing in-home Physical Therapy services as part of a dedicated hospital-based Home Health team. This contract opportunity is ideal for an experienced Physical Therapist who thrives in a collaborative, patient-focused environment and is eager to contribute to a growing program.


Schedule/Hours:

• Day shifts
• Occasional Saturday rotation for orthopedic patients
• Holidays off

Qualifications:

• Minimum 2 years of Physical Therapy experience, preferably in a hospital or home health setting
• Current and active California Physical Therapy license
• Basic Life Support (BLS) certified
• Comfortable with SOC visits, especially for orthopedic patients without nursing orders
• Working knowledge of EMR systems; experience with Netsmart is a plus
• Valid driver's license and willingness to travel within most of Tehama County (typical mileage around 40 miles per day)

Key Responsibilities:

• Provide skilled physical therapy evaluations and interventions for a caseload including orthopedic/joint replacements and geriatric patients with complex co-morbidities (e.g., CHF, COPD)
• Maintain daily visit ratios (approximately 5 visits/day)
• Complete thorough documentation using provided laptops for EMR
• Collaborate with an office-based Intake RN, Manager, and supportive care staff including nurse aides
• Contribute to a dynamic team focused on expanding patient access and quality care
