Clinical Intake Coordinator - LPN

Job Description The Clinical Intake Coordinator, LPN facilitates the Home Health & Hospice referral process by receiving referrals from multiple referral sources within and without the Saint Luke's Health System. Determines need for additional information from referral sources. Communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals. Prioritizes and triages hospice referrals. Ensures regulatory compliance is met during initial screening and intake process. Employees are required to work on-site 3–4 days per week, with the option to work remotely 1–2 days per week after orientation. Remote days are scheduled on a rotating basis to ensure adequate office coverage. Work arrangements are subject to change based on business needs. Responsibilities Facilitate Home Health & Hospice referral process. Determine need for additional information from referral sources. Communicate acceptance or decline of referrals. Maintain onsite presence 3–4 days per week; coordinate remote days as required.
