Responsibilities:

• Provide direct patient care, including administering medications, wound care, and other treatments as ordered by physicians

• Develop and implement individualized care plans in collaboration with interdisciplinary team members

• Monitor and document patient progress, reporting any changes or concerns to physicians and other healthcare professionals

• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions and patient satisfaction

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Demonstrate a commitment to patient-centered care, empathy, and compassion in all interactions

About the Role:
