Temporary Float Peer Navigator, Street Medicine (Hollywood/San Fernando Valley)

Overview At Healthcare in Action, we serve patients experiencing homelessness through an innovative street medicine approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems. Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks. The Peer Navigator supports members who are experiencing or are at risk of experiencing homelessness with managing their own health in the Los Angeles North and San Fernando Valley, CA area. Assist in motivating behavioral changes in patients to improve health outcomes through education, peer support, housing and resource navigation, and the relaying of shared experiences. This is a temporary assignment for approximately 5 months in duration. Responsibilities Build rapport with members by communicating personal experiences with a goal of increasing the likelihood of positive behavior changes. Coach members to create and implement strategies that minimize risks associated with identified common health conditions and behaviors. Connect members to appropriate programs to address barriers to care and to enhance compliance. Link members to local, county and state services and resources. Follow up with members and serve as a member advocate, delivering medications and other supplies as needed. Conduct regular outreach to onsite patients at the Nav Center as well as to off‑site patients to provide support for medical needs. Provide accompaniment and support to patients for their medical, dental, or other appointments. Collaborate actively with staff of the Navigation Center and with HIA team members for care management of patients. Introduce tools to promote self‑management and self‑efficacy. Complete all required organization and health‑plan specific documentation in a timely fashion, including entering all patient touch points and encounter notes in HIA’s electronic health record system, updating members’ Plan of Care after every encounter, documenting information in the care‑management system during phone calls, submitting a monthly audit checklist assessment, individual care plans, and updating member profiles in the Coordinated Entry System/HMIS, and scheduling encounters based on member acuity. Work under the supervision of a Social Worker to ensure proper escalation of cases and coordination of care. Actively support the achievement of HIA’s vision and goals, particularly housing placement for unsheltered members. Serve as a patient‑facing role; other duties as assigned. Qualifications G.E.D. or high school diploma, required, plus at least 3 years of work experience in relevant fields. Bachelor’s degree preferred, plus 1–2 years of work experience. Work experience to include direct experience, lived experience, training or volunteer work with patients experiencing homelessness, as well as those with active mental health and substance use conditions. Relevant experience in similar community settings or outreach with disadvantaged groups will be considered. A comparable combination of education, experience, and/or training will be considered equivalent to the required education. Ability to work in a dynamic, outdoor environment. Ability to work independently and as part of a team. Ability to organize effectively and prioritize multiple competing tasks. Excellent oral communication, strong interpersonal skills, and the ability to articulate personal experiences. Reliability of transportation. Computer proficiency, including navigating multiple systems such as online timesheets, call center software, and HMIS/Coordinated Entry System (preferred). Valid driver’s license, clean driving record, automobile insurance, and reliable transportation (travel 50% or more of the time). Benefits Base salary range: $21.78–$33.02 per hour.
