Job Summary Alignment Health is seeking an organized, compassionate, and motivated bilingual Spanish Health Coach & Educator to join the interdisciplinary Care Anywhere team in San Gabriel Valley / Southeast LA, California. This field‑remote position involves conducting home and virtual visits to develop a wellness coaching relationship with patients, caregivers, and families. Schedule Monday – Friday, 8:00AM – 5:00PM Pacific Time. Must conduct 6–8 home visits per day. Mileage reimbursement is provided. General Duties / Responsibilities Contact patients eligible for Alignment’s Care at Home program and provide wellness coaching to reduce or eliminate high‑risk behaviors. Collaborate with advanced practice clinicians and/or social workers to develop an action plan. Encourage adoption of habits that promote a higher quality of life. Deliver a high level of patient service and increase member retention and satisfaction. Assist patients with accessing community resources and social services to address social determinants impacting health and wellbeing. Develop and distribute health education materials to identified patients, caregivers, and/or families. Coordinate care with other Alignment team members, including health‑related vendors, for optimal patient care. Minimum Requirements Experience: Minimum 1+ year as a wellness or health coach or educator. Preferred: senior care experience. Education: High School Diploma or GED. Associate’s degree or two additional years of experience acceptable in lieu of a degree. Preferred: Bachelor’s Degree. Specialized Skills: Ability to communicate positively, professionally, and effectively; provide leadership, teach, and collaborate. Solid knowledge of specific diseases, lifestyle‑related topics, nutrition, and exercise. Knowledge of and experience applying the Stages of Change or a similar behavior‑change model. Excellent interpersonal skills; ability to foster trust and respect among customers and coworkers. Ability to work independently and remotely; handle protected health information while maintaining absolute confidentiality and privacy in accordance with HIPAA regulations and AHP policies. Proficient computer skills including Microsoft Office (Excel, Word, PowerPoint). Ability to work a flexible schedule. Strong written and oral communication skills; ability to establish and maintain constructive relationships with diverse members, management, employees, and vendors. Mathematical skills: perform calculations and simple statistics correctly. Reasoning skills: prioritize multiple tasks, advanced problem‑solving, use advanced reasoning to define problems, collect data, establish facts, draw valid conclusions, and design, implement, and manage appropriate resolution. Effective problem‑solving, organizational, and time‑management skills for fast‑paced environments. Preferred: Bilingual English and Spanish. Licensure: Current and valid California Driver’s License and reliable transportation. Work Environment & Physical Requirements Reasonable accommodations may be made.
