Case Worker

This position is located in the District of Columbia Government (District), Fire & Emergency Medical Services Department (FEMS), Office of the Medical Director, Mobile Integrated Health Team (MIH Team). The mission of the District of Columbia FEMS is to preserve life and promote health and safety through excellent pre-hospital treatment and transportation, fire prevention, fire suppression, rescue activities, and homeland security awareness. Duties and Responsibilities Acts as the primary professional point of contact between the MIH field team, private Primary Care Providers (PCPs), and hospital case management departments. Coordinates the transition of care for clients from the acute hospital settings back to community-based clinical support. Manages a diverse caseload of residents across all demographics who frequently access emergency services. Performs root-cause assessments to identify social determinants of health and develops individuated care plans to direct clients to appropriate nonemergency resources. Serves as a critical liaison for clients affected by the opioid crisis. Provides specialized outreach and coordination for patients qualifying for or initiated on Medication-Assisted Treatment (MAT) {i.e. buprenorphine, methadone, etc.} to ensure a “warm handoff” to long-term behavioral health or addiction treatment centers. Navigates complex insurance landscapes, with a specific focus on DC Medicaid and Medicare. Assists clients in resolving eligibility issues, establishing “medical homes,” and securing necessary medications or medical equipment. Provides targeted case management for residents with chronic health conditions. Ensures clients have the necessary specialty referrals and home-based supports to manage their health effectively outside of an emergency setting. Maintains meticulous records of all patient interactions within departmental tracking systems. Utilizes Excel and CAD-based tools to ensure data is accurate, timely, and organized to prevent duplication of services. Qualifications and Education Degree: behavioral or social science; or related disciplines appropriate to the position. OR Combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. OR Four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. In addition to candidates must have specialized experience that equipped the applicant with the knowledge, skills, and abilities to perform successfully the duties of the position, and that is typically in or related to the work of the position to be filled. To be creditable, one (1) year of specialized experience must have been equivalent to at least the next lower grade in the normal line of progression. License and Certification Incumbent must have a valid driver’s license. The work is primarily performed in the community with clients and in an office setting. As a condition of employment, you must successfully pass a criminal background check, traffic record check (as applicable), and a pre-employment drug and alcohol test. Acceptance of this protection-sensitive position confirms your understanding that you may be disqualified from employment if you are in possession of, or are impaired by, marijuana at the time of pre-employment drug and alcohol testing. For the duration of your tenure, you must comply with requests (if needed) for periodic criminal background checks as well as reasonable suspicion, post-accident or post-incident, and return-to-duty and follow-up drug and alcohol screenings. Residency Preference: Applicants claiming “Residency Preference” will be required to maintain residency in the District of Columbia for a minimum of seven (7) years. Failure to do so may result in forfeiture of employment.
