Board certified. Florida Medical License. DEA License. Required Qualifications Licensed to practice medicine in accordance with Chapter 458, Florida Statutes plus four years of appropriate experience. Completion of a residency program may substitute for the required experience. Preferred Qualifications Specialty License or Certification; Board certified or Board Eligible in specified area of medical specialty.
