Community Resource Navigator for Independent Living
Telligen
Telligen is seeking a Community Liaison / Resource Navigator in Oak Brook, IL to connect individuals with community resources and provide non-clinical support to care managers. You will develop partnerships with community resources, facilities, state agencies, and community entities to improve access to housing and support services. The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition to community living. #J-18808-Ljbffr Telligen
