Hospice RN Case Manager — Quality of Life Advocate

Hospice Alliance Inc

Hospice Alliance Inc in Pleasant Prairie, Wisconsin, is seeking a full-time RN Case Manager to plan and deliver care for terminally ill patients. The RN will coordinate care in various settings, including patients' homes and skilled nursing facilities, ensuring the highest level of comfort and quality of life. The ideal candidate will be a graduate of an accredited nursing school with a valid Wisconsin RN license. Experience in hospice or palliative care is preferred.
