Job Description

Schedule & Structure:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based visits to patient residences within an assigned territory
• Manage a caseload equating to 30 points per week (typically 35 visits daily)
• Electronic documentation using Home Care Home Base (HCHB)

Qualifications:

• Active, unrestricted RN license for the state
• Minimum one year of recent experience in home health (field preferred)
• Advanced knowledge and application of OASIS assessments
• Experienced user of Home Care Home Base (HCHB) electronic system
• Reliable vehicle, valid drivers license, and current auto insurance

Primary Responsibilities:

• Complete detailed OASIS assessments and devise individualized care plans
• Deliver skilled nursing: wound care, medication administration, IV therapy, management of chronic conditions
• Closely track patient progress and changes in condition, promptly communicating updates to physicians and the care team
• Provide education to patients and families concerning disease management, medications, and preventative health
• Ensure timely, accurate documentation in HCHB
• Collaborate with multidisciplinary team members for coordinated, comprehensive care
• Attend case conferences, team meetings, and pursue continuing education
• Maintain compliance with all agency and regulatory standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
