Physical Therapist (PT) – Home Health Visits Bayada Home Health Care is seeking an experienced Physical Therapist to join our Essex County team, providing home health visits to adult and geriatric clients throughout the area. As a Home Health Physical Therapist, you will manage your own schedule while delivering personalized, one‑on‑one care that helps clients recover safely in the comfort of their homes. Perform comprehensive evaluations, including assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities. Develop individualized treatment plans based on patient needs and physician orders. Implement evidence‑based therapeutic interventions, including therapeutic exercise, gait training, balance activities, and mobility training. Provide integumentary and wound prevention interventions as clinically appropriate. Collaborate with nursing and interdisciplinary team members to coordinate patient care. Educate patients, caregivers, and family members to promote safety, independence, and successful recovery at home. Continuously assess patient progress and modify treatment plans as needed. Accurately document evaluations, interventions, and patient progress using electronic medical records and tablet technology. Qualifications Current New Jersey Physical Therapist (PT) license. Minimum of one year of recent clinical experience as a licensed Physical Therapist. Graduate of an accredited Physical Therapy program. Ability to work independently and effectively manage time. Strong communication, organizational, and problem‑solving skills. Electronic medical record (EMR) experience preferred. Prior home health experience is a plus but not required.
