In this flexible role, you will provide skilled nursing services to clients in their homes, supporting safe, high-quality care and promoting independence.Working under the supervision of a Registered Nurse (RN), you will implement individualized care plans, monitor client conditions, and support families throughout the care journey.Why Join SmithLife HomecareFlexible PRN scheduling to support work-life balanceOpportunity to work with a highly respected, mission-driven organizationCollaborative clinical team with strong RN leadershipExposure to diverse cases including post-acute recovery and aging-in-place supportCommitment to excellence in dementia care and senior servicesKey ResponsibilitiesDeliver skilled nursing care in accordance with the established Plan of CareAdminister medications, injections, and treatments as prescribedMonitor and document client condition, including vital signs and response to careObserve and report changes in physical, emotional, or cognitive statusAssist with clinical procedures including wound care and infection preventionSupport clients with activities of daily living as neededEducate clients and families on care needs and disease managementMaintain accurate, timely documentation in accordance with agency standardsCollaborate with RNs, care coordinators, and interdisciplinary team membersFollow all infection control protocols and agency policiesQualificationsActive LPN license in Maryland and/or Washington, DC (in good standing)Graduate of an accredited practical nursing programMinimum 3 years of clinical nursing experience preferredStrong clinical judgment, communication, and documentation skillsExperience in home care, geriatrics, or chronic disease management preferredAbility to work independently and adapt in a home-based settingPhysical RequirementsAbility to lift, assist, and reposition clients as needed (50+ lbs)Frequent standing, walking, bending, and kneelingFull use of hands and arms for clinical tasksSchedule & StatusPRN / Per Diem (As Needed)Variable hours based on client needs, including potential evenings, weekends, or short-term assignmentsCompensationCompetitive hourly rate based on experienceOpportunities for additional hours based on performance and availabilityAbout SmithLife HomecareSmithLife Homecare is a licensed, bonded, and insured home care agency serving Maryland and Washington, DC.
