Licensed Vocational Nurse (Home Health) FLSA Status: Non-Exempt Reports To: Director of Patient Care Services (DPCS) /ADON Location: Field-based (Home Visits) – Various Locations in California Currently Servicing the Following Areas: Fresno and surrounding areas Job Summary The Licensed Vocational Nurse (LVN) provides direct nursing care to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with the care plan. The LVN supports patients and caregivers by implementing prescribed nursing interventions, observing and documenting changes in condition, and ensuring patient safety while complying with California state regulations and Medicare Conditions of Participation. Essential Duties and Responsibilities Provide direct patient care under the supervision of a Registered Nurse (RN) as ordered by the physician. Perform nursing procedures including but not limited to wound care, catheter care, medication administration, labs and vital sign monitoring. Observe and report changes in patient condition to the supervising RN and physician. Document nursing care, observations, and interventions in the electronic health record (EHR). Educate patients and caregivers on basic healthcare practices and self-care techniques. Assist with patient teaching as instructed by the RN. Support patients with activities of daily living as needed within scope of practice. Participate in in-service training, case conferences, and mandatory staff meetings as required. Provide emotional support to patients and families in coordination with the care team. Documentation and Compliance 24-hour policy-Complete and submit all documentation and care notes by the end of each shift or as directed. Maintain compliance with Medicare, ACHC, HIPAA, and California Board of Vocational Nursing requirements. Adhere to mandatory reporting laws, including any suspected abuse or neglect. Adhere to compliance with all agency policies and follow the plan of care and scope of LVN practice in the state of California. Mandatory Reporting As a mandated reporter under California law, the LVN is required to report any suspected abuse, neglect, or exploitation of patients, including elder or dependent adult abuse, to the appropriate authorities. Training and reporting procedures will be provided during orientation. Work Environment This is a field-based position with regular travel to patient homes. LVNs may be exposed to different home environments within their assigned service areas. Weekend and Holiday Availability Due to patient care and operational needs, employees are required to work a minimum of one major holiday per year and participate in a rotating weekend schedule to ensure consistent and equitable coverage. Driving Requirements This position requires regular travel to patient homes. Employees must have a valid driver’s license, reliable transportation, and current auto insurance that meets California’s minimum liability coverage requirements. Proof of insurance must be provided upon hire and may be requested at any time during employment. Qualifications Valid and active California Licensed Vocational Nurse (LVN) license. Minimum of one (1) year of clinical nursing experience, preferably in home health or community health. CPR certification (American Heart Association required). Reliable transportation and valid California driver’s license.
