Primary Responsibilities Initiates physical therapy evaluation according to physician orders and Plan of Care for the purpose of symptom control or to enable the patient to maintain activities of daily living and basic functional skills. Documents in the patient’s clinical record each visit and gives a report including the patient’s reaction to treatment and any changes in the patient’s status, including comfort level and follow‑up to physician and Administrator using the Care Management process. Participates in the Interdisciplinary Group and provides applicable input for the patient’s Plan of Care as needed. Instructs and informs participating members of the Interdisciplinary group, the patient, the physician and the family about functional limitations and progress toward goals as defined by the Plan of Care daily or no less than two weeks’ time. Prepares a written discharge summary when physical therapy services are discontinued, with a copy retained in the patient’s clinical record and a copy forwarded to the patient’s physician upon discharge from the physical therapy service. Instructs, when appropriate, patient, family and other members of the hospice team in certain phases of physical therapy in which they may work with the patient. Required Qualifications Current Physical Therapy licensure in state of practice. Current CPR certification. Current driver’s license and vehicle insurance, or access to a dependable vehicle or public transportation. Ability to function in any home situation regardless of age, race, creed, color, sex, disability or financial condition of the client. Benefits Pay is based on several factors including local labor markets, education, work experience and certifications. The hourly pay for this role will range from $34.23 to $61.15 per hour based on full‑time employment.
