In this role, your responsibilities will include:

• Conducting thorough evaluations of patients in diverse clinical settings, either inpatient or outpatient, and crafting tailored treatment plans to meet their unique needs.
• Implementing physical therapy interventions that focus on enhancing mobility, strength, balance, and overall functionality.
• Collaborating closely with an interdisciplinary teamincluding nurses, physicians, case managers, and other rehab specialiststo ensure the best possible patient outcomes.
• Educating patients and their families about therapeutic objectives, engaging in discharge planning, and guiding them through home exercise protocols.
• Meticulously documenting evaluations, treatment sessions, and patient progress in alignment with facility and regulatory standards.
• Participating in care team rounds and interdisciplinary meetings, promoting coordinated and comprehensive care.

To excel in this role, you will need:

• A Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution.
• Valid state licensure as a Physical Therapist.
• CPR certification is preferred, along with any specialized certifications in areas such as neurology or geriatrics, although they aren't essential.
• Experience in the field is strongly preferred, with an ability to manage patients facing complex medical and mobility challenges.
• Excellent communication and collaboration abilities.
• Familiarity with digital charting and clinical documentation systems.
