Responsibilities
- Plan and conduct segments of treatment programs to restore and improve physical function, while coordinating with an interdisciplinary team.
- Conduct initial and ongoing patient evaluations and establish individualized treatment plans using evidence-based physical therapy techniques.
- Provide therapy interventions following established protocols and document patient progress in a timely and accurate manner.
- Develop individualized care and discharge plans; communicate plans with patients, families, and other members of the care team.
- Supervise assigned assistants and ensure appropriate supervision and supervision documentation.
- Maintain compliant, comprehensive clinical documentation and participate in quality improvement activities as required.

Requirements
- Doctor of Physical Therapy (DPT) degree; licensure restrictions apply as noted (only if license obtained on or after January 2016; or foreign equivalent).
- Physical Therapy licensure in all applicable states of practice.
- CPR/BLS certification.
- Valid Driver’s License and must be at least 21 years old.
- At least two years of recent experience in the field.
- EPIC EMR experience required.
- Current nursing license per the state of assignment.
