Job Description

We are currently seeking a qualified candidate for an Acute Care Case Management position. The role is expected to commence as soon as possible, with a scheduling structure of Day shifts, 5 days a week, working from 08:00 AM to 04:30 PM.

Position Details

Shift: Day 5x8-Hour (08:00 - 16:30)
Location: City, State
Start Date: ASAP
Floating Expectation: No
First-Time Traveler: Not allowed; must be a seasoned traveler

Requirements

Experience: Minimum of 2 years in Acute Care Case Management or Discharge Planning, with hospital experience required. Behavioral health experience is not applicable.
Ratios: Patient ratio of 1:25 on days, 1:45 on weekends.
Weekend Requirement: Every other weekend to meet departmental needs.
License: California state license required. Pending licenses accepted, but candidate must be licensed upon starting.
RTO Restrictions: Maximum of 7 days.

Daily Census: Approximately 25 patients.

Skills and Competencies

Experience with patient assessment, care coordination, discharge planning, and utilization review.
Ability to organize and facilitate family meetings.
Participation in the development of treatment plans.
Familiarity with HMOs, IPAs, and knowledge of various patient support systems.
Competence in arranging Durable Medical Equipment (DME), Skilled Nursing Facility (SNF) placements, and ambulance transport independently.
Knowledge of addressing issues such as substance abuse, mental health crises, and social services processes.
Proficient in computerized documentation and relevant charting systems (Epic preferred).

Additional Information

Travelers may be expected to float to nearby facilities if required.
Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).
Pre-approved time off must be communicated upfront during the submission process.
