Seeking a compassionate and experienced Registered Nurse to provide high-quality hospice care to patients in their homes. This role focuses on delivering patient-centered care, participating in interdisciplinary collaboration, and ensuring comfort and dignity for patients and their families throughout the hospice journey. Seeking a compassionate and experienced Registered Nurse to provide high-quality hospice care to patients in their homes. This role focuses on delivering patient-centered care, participating in interdisciplinary collaboration, and ensuring comfort and dignity for patients and their families throughout the hospice journey. Schedule & Work Details:
- Location: Baton Rouge, LA and 50-mile radius
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Guarantee: 36 hours
- On-Call/Weekend Requirement: Yes
- Mileage Reimbursement: Yes, between patient visits (first to last patient)
- Pay Rate: $51 to $53 per hour
- Provide day-to-day skilled nursing care to hospice patients in their homes
- Perform patient assessments and monitor changes in condition
- Participate in IDG (Interdisciplinary Group) meetings and complete IDG documentation
- Coordinate and manage patient care plans in collaboration with the hospice care team
- Order medications, medical supplies, and durable medical equipment (DME) as needed
- Educate patients and families on disease progression, symptom management, and end-of-life care
- Maintain accurate, timely, and compliant documentation, including EMR usage
- Ensure compassionate support for patients and families during end-of-life care
- Active Registered Nurse (RN) license in the state of Louisiana
- Associate of Science in Nursing (ASN) required
- Prior hospice or relevant nursing experience required
- Strong clinical, communication, and documentation skills
- Ability to work independently and manage a field-based schedule
- Experience with Homecare Homebase (HCHB)
- Strong organizational and time management skills
- Compassionate approach with a focus on patient dignity and comfort
