Telephonic Triage And Document Review ServicesProvides telephonic triage and document review services that require clinical judgement and interpretation and acts as a liaison between the patient, care team(s), and their primary care provider. The core Success Factors include: Instill Trust and Value Differences Patient and Community Focus and Collaborate.Responsibilities:Reviews and triages telephone encounters, calls, and messages from patients and de-escalates or escalates to the appropriate care team or clinicianPrioritizes urgent medical triage to patientsProvides timely assessment for smart scheduling into clinician scheduleReviews lab results, imaging results, consultation notes, and clinical summaries and identifies those that need to be directed to the respective clinicians for reviewEffectively communicates with appropriate disciplines involved in the care of patients, and provides information and recommendations to providersDevelops professional working relationships with health care providers and facilities, generating an open flow of information and support of home care goalsPromotes effective working relationships with nursing personnel, the management team and health care providersDetermines individual patient needs for services, referrals and training and provides patient education as necessary on plan of careAttends required meetings and huddlesSeeks guidance and resources, when necessary, in care and treatment of patientsProvides guidance and direction to colleagues and acts as a resource/mentor for nonclinical staffCoordinates team based care in the absence of the Program ManagerComplies with federal and local confidentiality laws, including HIPAA, ensuring patient privacyAdheres to Brown Health Medical Group Primary Care guidelines and policies for protecting patients' demographic, clinical and financial informationPerforms other job-related duties as assignedMinimum Qualifications:Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.Education and Experience: Graduate of an accredited school of nursing. Current Rhode Island RN license in good standing. 3-5 years of appropriate nursing experience.Knowledge, Skills, and Ability: Capacity to collaborate with colleagues and teammates as necessary to ensure quality work. Ability to work both on a team and independently, with the capability to self-direct. Ability to use technology with proficiency in electronic medical record (EMR) platforms and Microsoft Office applications such as: Outlook, Excel, and Word. Ability to function well in a high-paced and at times stressful environment. Capacity to multi-task, think critically and problem solve. Clear assessment, creativity, judgment, and decision-making skills. Ability to take initiative, organize, prioritize, and follow through with work assignments assuring fulfillment of plans and goals. Excellent communication skills – both verbal and written. Ability to maintain quality, safety, and/or infection control standards. Ability to perform standard clinical procedures according to established protocols.
