Family Medicine Physician - Health Equity for Adolescents in the Legal System (HEAL)

Position Summary Physician Affiliate Group of New York (PAGNY) is offering a full‑time Family Medicine Physician – Health Equity for Adolescents in the Legal System (HEAL) position with the Administration for Children’s Services (ACS). The role provides 24/7 medical care across two secure detention sites, two school‑based health centers, and limited pharmacy services at seven non‑secure sites. Responsibilities include comprehensive screenings, vaccinations, diagnostic procedures, treatment planning, and referrals to off‑site care as needed. The position is part of NYC Health + Hospitals’ initiative to deliver equitable, high‑quality care to young people in custody. Responsibilities Provide medical care to ACS adolescents, ensuring safety and security measures. Take relevant patient history, identify patient needs, and make diagnoses. Develop and implement treatment plans using diagnostic data. Perform comprehensive well‑care examinations, including physical and gynecological exams. Write prescriptions, inform patients about medications, obtain parent/guardian consent when needed. Complete medical forms and documentation related to patient care. Make and track referrals to other agencies and providers, ensuring follow‑up care. Assist with phlebotomy as needed to support patient flow. Administer immunizations. Distribute, refill, discard, and discontinue medications for patients. Complete court orders and other ACS mandated paperwork. Evaluate and treat ACS staff only on an emergent basis. Provide health education on preventive medicine, chronic disease counseling, parenting, prenatal care, nutrition, family‑planning, medication usage, and stress management. Assist with administrative duties: Participate in patient outreach. Participate in case conferences. Assist Medical Director in developing and implementing medical protocols and forms. Initiate or participate in research related to health care. Qualifications Current New York State physician registration. Board certification in Family Medicine. Minimum three years of related experience in family, pediatric, or adolescent urban populations. Must pass all appropriate screenings for a secured setting with young people. Spanish language proficiency preferred (not required). Compensation & Benefits Annual base salary: $220,000* (board‑certified) based on a 40‑hour work week. The estimated annual total value of the compensation package is $242,000** (board‑certified), which includes the baseline salary, 401(k) contributions, and other benefits. 401(k) company contribution: 3% of base earnings immediately vested; 7% after one year of service.
