Registered Nurse- Home Care

Position Overview Sentara Health is seeking a qualified individual to join our team as a Registered Nurse . This PRN position offers flexible hours and is based at Sentara Home Care Services in Williamsburg, VA, with travel within a 40‑mile radius. The work shift is First (Days). Responsibilities The Direct Care Registered Nurse – Home Health and Hospice develops, implements, and evaluates care plans for specific patient populations. The RN manages a patient caseload by coordinating and collaborating with multidisciplinary team members to facilitate integrated and comprehensive care, while maintaining clinical knowledge to meet standards set for specific clinical areas.
