Location: GA Wellness - Elk River, MN Salary Range: $35 - $50/Hour (Based on Experience) Job ID: 3202 As part of Guardian Angels' continued growth, this is a newly created opportunity as we transition from a contracted therapy model to a program supported by Guardian Angels. This is a unique opportunity to join a newly formed team and help establish process, culture, and clinical excellence from the start. Position Summary Provides and oversees Speech‑Language Pathology services for residents/clients, including evaluation, treatment, development and execution of therapy plans, and completion of all required documentation and communication. Works under physician orders and follows established standards of speech‑language pathology, along with all facility policies, procedures, and state regulations. Provides supervision to Rehabilitation Aides as part of the care team. Services are provided in accordance with the mission of Guardian Angels. Job Duties Optimize communication, cognitive and swallowing skills of resident/client by performing or supervising therapy staff in the following activities: initiating referrals; evaluating and treating appropriate patients; complying with Minimum Data Set procedures; effectively participating in the care planning process; appropriately documenting progress toward identified objectives and communicating patient status and needs to the resident/client, the resident/clients family, the staff and other professionals as appropriate. Conducts screenings of patients at regular intervals to determine need for intervention/treatment. Establishes individual patient schedules in conjunction with other team members. Develops treatment plans by identifying a problem list, including long and short‑term goals and methods to achieve identified goals. Coordinates treatment plan with resident/client, family, and other team members. Implements and conducts treatment as outlined in the treatment plan. Monitors patient’s response to intervention and modifies treatment as indicated to attain goals in accordance with the physician’s orders. Develops appropriate home or community programming to maintain and enhance the performance of the resident/client his/her own environment. Documents results of resident/client assessment, treatment, follow‑up, and termination of services. Adapts treatment to meet the individual needs of the resident/client. Recommends and obtains appropriate equipment to improve resident/client communication. Instructs resident/client, family, and staff in the use of augmentative communication devices. Instructs resident/client, family, and staff in appropriate speech language procedures or home program to allow carryover of therapeutic remediation and goals in daily activities. Participates in discharge planning. Supervises students and volunteers. Reviews the quality and appropriateness of the total services delivered and of individual speech language pathology programs for effectiveness and efficiency, using predetermined criteria. Participates in resident/client, family, and staff education. Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient. Adheres to Facility policies and procedures regarding documentation of speech language pathology services. Attends in‑service training and facility meetings as required. Provides in‑service education to the members of the resident/client care team and to members of the community. Assumes responsibility for ongoing continuing education and professional development. Submits billing, payroll, and expense documentation in an accurate and timely manner. Assists in the collection of statistics as directed. Provides treatment in accordance with established standards of practice, department procedures, and efficiency standards. Consistently demonstrates sound judgment in the evaluation, planning, implementation, and follow‑up of the patient therapy programs. Maintains positive relationships and rapport with coworkers, resident/client, family members, and facility personnel. Reports to work on time and coordinates the schedule to achieve maximum efficiency during the assigned shift. Maintains a safe and clean work area and adheres to facility/corporate safety standards. Assists with and participates in Continuous Quality Improvement Program as required. Adheres to established confidentiality standards as required by Facility concerning community and patient information. Projects a positive and professional image at all times. Complies with all Human Resources policies and policies set forth by the organization. To assist in providing a dignified, respectful, resident‑centered environment for residents. Follows universal precautions, facility infection control guidelines, sanitation and safety standards of practice in all work and activities. Current standards, guidelines, state and federal regulations. Uphold confidential information as required by Guardian Angels policy and procedure and as related to HIPAA. Uphold the mission, vision, and values of Guardian Angels. Performs other duties as assigned. Qualifications Education/Experience Master’s degree in Speech Language Pathology from an accredited program. Certificate of Clinical Competency and licensed in the state of Minnesota. Experience One to three years of prior experience in a rehabilitation setting such as acute care, inpatient rehab, outpatient clinics, SNF, long‑term care, assisted living, memory care, or home health. Knowledge of State and Federal Regulations. Strong problem‑solving and critical‑thinking skills are essential. Excellent organizational skills. Ability to multitask, prioritize, and work independently. Excellent verbal and written (read, write) communication skills. Proficient with Microsoft Office 365 required. Net Health software experience required. Familiarity with other healthcare software, such as PointClickCare (PCC), is a plus. Maintains good attendance and punctuality.
