Facilitator, 

Day Habilitation Program

 

Summary

Observing and monitoring all social and recreational activities while promoting personal growth, independence, and choices for Day Habilitation Participants.

Position Type

Non-Exempt / Full-Time

Essential Functions

1. Provide opportunities for individuals to freely choose community-based activities 
2. Support and promote a person-centered culture
3. Assist individuals with building and maintaining community relationships
4. Empower individuals to be as independent as possible in the areas of leisure, social, hygiene and life skills
5. Advocate for community inclusion 
6. Educate participants in self-advocacy
7. Responsible for solicitating feedback from participants (their interests, their likes, their dislikes, etc.) for the purpose of incorporating new opportunities
8. Maintain and follow all safety precautions, ensuring a safe environment 
9. Provide support to program individuals’ emotional and physical wellness, based on their preferences
10. Provide transportation as needed
11. Provide support and assistance to participants as needed
12. Daily implementation of the Geaux Groups 100% Community Based BAND calendar 
13. Assist participants with setting community life engagement goals 
14. Maintain daily documentation—Community Life Engagement Activity Log
15. Report any significant concerns to supervisor
16. Maintain training requirements
17. Follow all guidelines & procedures related to LaSRS and ADP.
18. Attend and participate in staff meetings
19. Other projects and duties as assigned

Supervisory Responsibility

None

Required Education and Experience

High school diploma or equivalent required. At least one year experience working with individuals with developmental disabilities is preferred. 

Other Requirements

Must maintain a valid Louisiana driver’s license, vehicle insurance, vehicle registration and vehicle inspection sticker. 

Physical Demands

While performing the duties of this job, the employee is frequently required to stand; walk; sit, use hands, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
