As a new Homecare Physical Therapist or Occupational Therapist, you’ll receive a thorough orientation and mentorship to be successful as well as strong management support. Our Therapists are responsible for managing a caseload of patients requiring home‑based therapy, primarily post‑acute or sub‑acute. Enjoy starting and ending your day from home! What You Will Do Promote/provide physical therapy and/or occupational therapy services in the home to assist the patient in achieving his/her optimal level of functioning. Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions. Teach patients/caregivers and help them to become independent with their treatments and self‑management of their chronic illness(es). Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians. Minimal weekend and holiday rotations. What You Will Need Current NYS PT or OT license and registration Minimum of one (1) year of experience as a PT or OT, preferably inpatient or homecare, is required Home care experience preferred, but not required Valid Driver’s License and Reliable Vehicle. This position provides mileage reimbursement.
