Role Description The Nurse Practitioner at MendWell Wound Care provides direct patient care focused on assessment, treatment, and ongoing management of acute and chronic wounds. In this full‑time, on‑site role based in Chicago, IL, the Nurse Practitioner performs comprehensive evaluations, orders and interprets diagnostic tests, develops individualized care plans, and prescribes appropriate therapies in alignment with clinical guidelines. Day‑to‑day responsibilities include conducting patient visits in facility or community settings, documenting clinical findings in the medical record, collaborating with physicians and interdisciplinary teams, and educating patients and caregivers on wound care, prevention, and self‑management. The Nurse Practitioner also participates in quality improvement initiatives, adheres to regulatory and infection‑control standards, and supports the continuous enhancement of MendWell’s mobile care model. Qualifications Active Nurse Practitioner license in Illinois and national certification (e.g., AGNP, FNP, or equivalent), with eligibility to prescribe medications. Clinical experience in wound care, geriatrics, long‑term care, or related fields, with strong skills in patient assessment, diagnosis, and treatment planning. Proficiency in electronic health records, clinical documentation, and use of mobile technology for care delivery and communication. Strong interpersonal, communication, and patient education skills, with the ability to work collaboratively within an interdisciplinary team and build trust with patients and families. Demonstrated ability to work autonomously in the field, manage a mobile schedule, and prioritize multiple patient visits while maintaining high standards of care. Knowledge of evidence‑based wound care practices, infection control, and applicable regulatory and compliance requirements. Master’s degree in Nursing from an accredited program; prior experience in a mobile, home‑based, or facility‑based practice is beneficial.
