Community Health Worker

Oak Street Health

Oak Street Health seeks a Community Health Worker to support Medicare patients in underserved communities. In this role, you will conduct outreach, build trusting relationships, and connect patients with primary care, social services, and preventive health resources. You’ll provide health education, assist with care coordination, schedule visits, and help address barriers such as transportation and benefits access. Working closely with clinicians and care teams in a patient-centered, team-based environment, you will play a key role in improving health outcomes and enhancing access to quality care.
