Matrix Providers is hiring a Nurse Practitioner – Family Medicine to join our team of talented professionals who provide health care services to our Military Service Members and their families at Evans Army Community Hospital located at Fort Carson CO.

• Employment Status: Full-Time
• Compensation: $63.65-$67.00/hour
• Schedule : Evans Army Community Hospital is open 24 hours a day, 7 days a week. Generally/ Current schedule is Monday through Friday, 7:30am - 4:30pm. Hours are subject to change, and may include nights, weekends, or home call if future need arises. The Contract employee shall work no more than 40 hours per week without prior authorization from the supervisor or department chief.

Requirements: Nurse Practitioner – Family Medicine must have and meet the following:

• Degree: Master's degree in nursing.
• Education: Graduate from an academic program, within the nursing specialty field of study that is accredited by a regional or national nursing accrediting agency recognized by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
• Certification:
  • Certified in Family Medicine as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners (AANP) or the recognized national nursing certification organization for the specialty area of practice as appropriate (for example, National Certification Board of Pediatric Nurse Practitioners and Nurses, American Nurses Association, National Certification Corporation for the Obstetric, Gynecologic and Neonatal Specialties).
  • Basic Life Support from the American Heart Association or American Red Cross.
• Experience: Have actively practiced as a Family Medicine-Nurse Practitioner for 24 months within the last 36 months, and are able to demonstrate current clinical competency. Current clinical competency is verifiable by clinical activity logs submitted by the previous medical institutions
• Licensure: Current, full, active, and unrestricted license as a Registered Nurse.

Core Duties:

• Provide a full range of outcome-based nurse practitioner services in accordance with privileges granted by the MTF (e.g., supervise and provide general screening and medical care and examinations of patients for routine, acute and chronic conditions involving any and all organ systems; provide immunizations; diagnose, treat, and counsel patients as indicated).
• Technically direct and teach other staff, provide evidenced-based educational lectures and participate in in-service training to staff members.
• Diagnose and treat patients with common acute conditions, chronic illnesses, or minor trauma within accepted protocols, clinical practice guidelines, Nurse Practice Acts, and/or in consultation with a physician.
• Promote preventive care and health maintenance including annual physicals, positive health behaviors and self-care through both formal and individual education and counseling.
• Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
• Recognize conditions that require isolation and ensure universal precautions are used in all patient encounters.
• Attend staff meetings to provide case discussion, continuing education, and continuous quality improvement and safety.

Clinic and Patient Information :

• Manage a fast-paced family medicine workload of approximately 20–23 scheduled patients per day, including 6–15 acute or high-acuity cases.
• Provide a mix of in-person and virtual care, with approximately four virtual appointments and four acute visits scheduled daily.
• Deliver well-child and well-baby examinations, acute care, follow-up visits, and care for new patients.
• Adapt to frequent schedule, staffing, and workload changes while maintaining timely, high-quality patient care.
• Complete clinical and administrative responsibilities, including documentation, patient messages, results review and follow-up, EFMP processes, email and Teams communication, safety reports, peer reviews, and continuing-education presentations.
• Participate in group meetings, quarterly provider meetings, and other assigned clinic activities.
• Work independently following an estimated four-to-six-week onboarding and training period.
• Collaborate effectively with clinic staff, leadership, and contract partners in a dynamic healthcare environment.
