Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0 shift schedule.

The Outpatient Physical Therapist is responsible for evaluating all assigned patients and developing, implementing, and modifying treatment plans based on the patient's needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services, ensuring the design of functional treatment programs that facilitate effective community reintegration. The therapist also serves as an educator to patients, families, coworkers, and students, while adhering to organizational and professional standards.

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document medical records accurately and in compliance with relevant guidelines.
• Provide care under signed, valid physician orders, ensuring insurance approval as needed.
• Participate in student programs, fostering a positive learning environment and supervision.
• Create an individualized professional development plan and pursue self-directed growth.
• Adhere to State Practice Act and maintain professional licensure in good standing.
• Engage in performance improvement initiatives to enhance the quality of services.

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
