This role specializes in the management of patient care, collaborating within a dynamic health setting. The position offers an opportunity to play a vital role in patient discharge planning and integration into facility workflows using Meditech and Midas systems.

Key Responsibilities of the RN - Case Management

• Coordinate and manage patient care plans effectively
• Conduct complex discharge planning
• Utilize Midas and Meditech systems for patient documentation
• Work collaboratively with healthcare teams, including RN CM and SW CM
• Maintain a patient ratio of at least 25:1 CM

Requirements of the RN - Case Management

• Valid RN license
• Experience with Meditech and Midas systems
• Proven experience in case management and discharge planning
