Registered Nurse Case Management Hospital

Job Summary:

The Case Manager is responsible for coordinating and managing patient care within Berkshire Medical Center to ensure quality, cost efficiency, and appropriate resource utilization. This role involves evaluating patient needs, facilitating transitions of care, and working collaboratively with multidisciplinary teams.

Responsibilities:

• 
  Review patients at admission and throughout their stay to determine appropriate levels of care and resource utilization.
  
  
• 
  Coordinate services and resources needed by patients, negotiating with providers and community resources as necessary.
  
  
• 
  Perform transition planning to arrange post-hospital care and monitor patient progress during hospitalization.
  
  
• 
  Participate in performance improvement initiatives to enhance patient outcomes and operational efficiency.
  
  
• 
  Maintain accurate documentation and data management supporting compliance with regulatory standards.
  
  
• 
  Demonstrate strong interpersonal and negotiation skills to collaborate effectively with providers, patients, families, and payers.
  
  

Qualifications:

• 
  Must possess 3+ years of clinical experience in acute care settings such as internal medicine, surgery, orthopedics, cardiology, or oncology.
  
  
• 
  Proven ability to analyze clinical data and present findings clearly and effectively.
  
  
• 
  Knowledge of regulatory agencies, including Joint Commission, DPH, and related standards.
  
  
• 
  Current Massachusetts RN license and active BLS certification required.
  
  
• 
  Excellent organizational, communication, and interpersonal skills essential.
  
  
• 
  Experience with Meditech Expanse and data management preferred.
