Registered Nurse Case Manager (RNCM) – Hospice Location: Pennsylvania – Patient Residences (Travel Required) Schedule: Full-Time | Monday–Friday, 8:00 AM–5:00 PM | Occasional evenings/weekends Salary: $85,000–$100,000/year (Above Industry Standard) Why Join Vitality Hospice & Palliative Care? Your Role as a Registered Nurse Case Manager (RNCM) As an RNCM, you’ll be a cornerstone of our interdisciplinary team, ensuring patients receive personalized, compassionate care. You will: Conduct patient admissions and comprehensive nursing assessments. Develop and implement individualized plans of care. Collaborate with hospice physicians, social workers, chaplains, and aides. Educate patients and families on symptom management and available resources. Supervise and coordinate care with home health aides and other team members. Maintain accurate, timely documentation in compliance with hospice standards. Participate in IDT meetings and promote a culture of empathy and excellence. Qualifications Current Registered Nurse (RN) License in Pennsylvania. Clinical nursing experience (hospice or home health preferred). Strong communication, organizational, and interpersonal skills. Valid driver’s license, reliable vehicle, and current auto insurance. Ability to travel within the assigned service area.
