Home Care Coordinator

To support the CNST Region, the full-time Texas Licensed Care Coordinator will provide exceptional customer service by managing healthcare services coordination between patients, family members, and care providers in a remote setting, while occasionally attending onsite training and meetings in Texas. Key Responsibilities Ensures a positive patient experience by focusing on satisfaction and resolution of inquiries Responds to customer inquiries via phone, email, and chat in a timely and professional manner Documents customer interactions and follows up to ensure issues are resolved satisfactorily Required Qualifications High school graduate or equivalent One year of experience in customer service, call center, or healthcare-related role Previous healthcare experience and familiarity with medical terminology Proficiency in Microsoft Office Ability to communicate effectively and clearly, both verbally and in writing
