Responsibilities

Spiritual Care Counselorsat DHHC are integral members of a multi-disciplinary health care team that are responsible for the provision of spiritual care services to patients and families/caregivers of the Agency, either directly or through coordination of care with other spiritual counselors.

The Spiritual Care Counselor will:

• Assure spiritual assessment of patients and families in the hospice program and appropriate services are rendered in a timely manner to meet regulatory guidelines.

• Provide direct spiritual care to patients and families/caregivers.

• Serve as liaison to community chaplains and spiritual counselors.

• Document direct services and ongoing communication with community resources.

• Provide funeral or memorial services for patients as requested.

• Provide bereavement support to families and caregivers for up to a year after the death of the patient.

• Perform hospice informationals for patients and families

Qualifications

• Graduate of Theological Studies from accredited university required.

• Experience with database software or electronic medical record software required.

• At least 1 unit of Clinical Pastoral Education required.

• Completion of Clinical Pastoral Education residency preferred.

• Two years of experience in health-related field preferred.

• Hospice experience preferred.
