Position: Registered Nurse Admission (Hospice)

Location: Denton County, TX

Job Type: Full-Time, Permanent

Schedule: Monday–Friday, 8:00 AM–5:00 PM

Salary: $85,000–$95,000/year (DOE)

Additional Compensation: $0.65/mile mileage reimbursement + $60/month cell phone reimbursement

On-Call: Required – On-call rotation

Travel: Required throughout assigned territory

Relocation Candidates: Accepted

Position Overview

We are seeking a compassionate and experienced Hospice Admissions Registered Nurse (RN) to join our team in Denton County, Texas.

The Hospice Admissions RN serves as the first clinical point of contact for patients and families beginning their hospice journey. This position combines strong clinical assessment skills with compassionate communication to ensure patients and families experience a smooth, supportive admission process.

The ideal candidate will have at least 1 year of hospice or senior care experience and a strong understanding of hospice philosophy, eligibility requirements, and end-of-life care.

Key Responsibilities

• Evaluate and assess patients to determine hospice eligibility.
• Complete comprehensive clinical assessments for prospective hospice patients.
• Guide patients and families through the hospice admission process with compassion and professionalism.
• Develop individualized plans of care for newly admitted patients.
• Provide skilled nursing care based on individualized patient care plans.
• Collaborate with physicians, nurses, social workers, chaplains, and other members of the interdisciplinary care team.
• Coordinate smooth transitions for newly admitted patients.
• Educate patients and families regarding hospice services, care plans, medications, and available support.
• Maintain accurate, timely, and thorough clinical documentation.
• Monitor patient needs and communicate significant changes to the appropriate members of the care team.
• Participate in the required on-call rotation.
• Travel throughout the assigned territory to provide patient care and admissions services.
• Perform additional clinical and administrative duties as assigned.

Qualifications

Required

• Current, active Texas Registered Nurse (RN) license.
• Minimum 1 year of Hospice or Senior Care experience.
• Strong understanding of hospice philosophy and eligibility requirements.
• Knowledge of end-of-life and comfort-focused care.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage daily responsibilities effectively.
• Ability to adapt to changing patient and family needs.
• Valid driver's license.
• Reliable transportation.
• Current automobile insurance.
• Must be at least 21 years of age.
• Successful completion of background checks.
• Professional references required.
